US Passport "Place of Issue"

I was completing the immigration information for our upcoming Azamara cruise and there is a question I have never encountered before. Azamara is asking for the "place of issue" of our passports and as far as I can tell there is nothing on our US passports that indicates a "place of issue". What should I list as the place of issue? Please do not respond unless you are a US passport holder that has previously sailed with Azamara or your name is Bonnie McLaird.

Just put " National Passport Center " onto the form.

I haven't sailed with Azamara, I am not Bonnie, but I do have a passport. On the photo page, lower right corner, it says "issuing authority." It used to say " Place of Issue." Issuing authority is 99.9% likely to be United States Department of State.

I have not cruised on Azamara yet, but I have cruised on both Celebrity and Royal Caribbean. I agree with the previous poster. I always use "United States Department of State". I have never had a problem.

I've always put United States Department of State.

"Washington, DC. USA" is our option. A non-US immigration official may not understand "Department of State."

I just did an Indian Visa and entered US Dept. of State. I think I did the same for a Vietnamese Visa. My last two passports said State Department, the one or two before that said National Passport Center, and before that it was the office that processed the passport (Miami, New Orleans, DC, some Consulate, etc.).

Just make sure it matches what your passport says in the lower right corner. That's all that anyone is likely looking at.

Hi Kevin,

I have put National Passport Center, and I believe I've also put San Francisco in the past. I suspect any of the suggestions by others will also work.

Thanks for the responses everyone. It appears that it really doesn't matter what we put down, as long as it sounds American 🇱🇷
