TIA cruisediva DOT com/permission_ltr.htm Here you go just print it off take to your local notary and have it filled out. It also includes consent for emergency medical treatment. We use our bank since they do not charge a fee.
GradUT Posted November 18, 2012 #3 Share Posted November 18, 2012 The above-referenced travel and medical authorization looks good. Just be sure that BOTH parents of the child sign it and have it notorized.
gusandgidgetsmom Posted November 18, 2012 #4 Share Posted November 18, 2012 Also make sure you have her insurance card as she probably is not on your medical insurance. We take our Grandchildren crusiing and have only needed the above docs and insurance cards.
cruisinlawn Posted November 18, 2012 Author #5 Share Posted November 18, 2012 perfect, thank you. Do the cruise lines require anything additional? I'll be checking with them as well. Thanks.
lillian marie Posted November 18, 2012 #6 Share Posted November 18, 2012 We have travelled on CCL with grandkids often. My daughter's a lawyer and she writes a letter stating permission for out of the US travel on Carnival for her child with dates listing the ports of call and gives medical permission. She and her husband sign it and it have it notarized. We have never had a problem with travel with our grandkids. We do buy the additional travel insurance for the kids too.
