Need help with Special needs form

No controversy on this thread. I filled out my Special needs form for my CPAP machine on our September cruise. There is nothing on the form to actually send the form. Do I have to send it via email. Never done this before. Thanks for the help.

I have called special needs because I had questions. I ended up giving them the info from the form and they said there was no need to return the form as they had my request noted.

I called becasue I could not tell if they had received or not. After my call, they sent a f/u email with my specific requests in writing and atttached it to our booking number. The email looked like this:

Hello,

 

We hope this email finds

you well. This is regarding your recent inquiry for the sailing on the

Celebrity Reflection on December 6, 2014.

 

As per our conversation, the reservation has been noted for Shirley's special needs. We will provide the commode chair, shower stool, distilled water and extension cord in the stateroom on embarkation day. We have also noted that Care Vacations will be delivering oxygen to the ship for Shirley.
