Health declaration form at check in

Does anyone have a copy of the form we have to fill in saying that we don't have any nasty bugs when we board?

It is much easier to fill in at home than at the terminal.

Questions 3 and 4 were no longer on the form I completed in October this year, so this link may be out of date.

Interesting, as already used 4 times this year without rejection, with number 5 for use on Monday.  

Okay, on Monday we'll ask for an additional form and scan it into laptop.

Thank you for this. I have raised a query at check-in previously why they are unavailable on the CP. Trying to fill these in whilst balancing items of luggage etc is a feat in itself. 

Will have 3 printed off and completed for check in on Monday.
