Me and my wife are arriving 2 days before cruise departure. 

 

A question about the Greek PLF form, section "Temporary Address"

Please list all the places you will be staying at during the 14 days following your arrival/departure

 

I have entered the address of the hotel we are staying pre-cruise. 

 

You have the option to check a box and fill in a second address 

Is this something you need to fill in? You can enter your cruise ship name, but what about city and postal code?

We arrived 1 day early too. I used the hotel for that, then used the port address for the second address for the city, code, etc. Didn’t put any other locations, they know where the cruise ship is going. Plus, they didn’t look at our form for 2 seconds when we arrived in Greece. They just wanted to see we had the QR code (so they know we did it), plus the proof of vaccine (or negative test). So I wouldn’t sweat it once you’ve got those 2 things on there.

When we filled out the form (back in July) we simply listed our cruise ship's name, Pireaus as the city and then used a random zip code for Pireaus (i.e 18540).  Our form was accepted without question.  We did not bother with any other destinations beyond our first night's hotel (Sofitel) and our ship.  Nobody ever ask to see that PLF form once it was checked at ATH...this despite the fact we spend a post cruise week island hopping (Naxos and Paros).   We did need to again show our vaccination certificates (to the Greek Coast Guard) before we could board our post cruise ferry from Pireaus to Naxos (a recent negative COVID test would have also been acceptable).

I also sailed out of Athens back in July and I put the name and address of the hotel (stayed 1 night pre-cruise) and I also put the name of the cruise line and the ship.
