I believe the HC form is used for special needs, like extension cords, sharps container, or other special needs. DUTRAVEL Posted December 2, 2021 #5 Share Posted December 2, 2021 I don't know whether or not you actually need to fill out the special needs form after you have booked an accessible cabin (I always do because I have a service dog). I think it would be a good idea to fill out the form (see Jim's link) and e-mail it to special_needs@celebrity.com just to have all your bases covered. When needed and booked a HC we saw no paperwork from anyone. We're asked if we would move to a HC cabin some years ago. It was a higher class. Moved and it was very nice. earlydiner Posted December 2, 2021 #7 Share Posted December 2, 2021 Each time you book an accessible cabin you must fill out the Special needs form, send it back to the special needs desk, and get approval for the use of the accessible room provided for you. We cruised on the Millennium in September and now on the Silhouette in January and they requested a new form. That's okay so that those who need an accessible cabin can get them. I called the number in the link and they said I didn’t need to fill anything out because my TA already confirmed that I needed that cabin. I only needed to fill it out if they didn’t or like you said, if I needed something special. They noted special assistance needed boarding and disembarking.
