Different kind of passport question

I have a question for those of you who have filled out the online documents and are using a passport. My husband and I have had our passports for a few years and they say New Orleans passport authority (even though we applied and live in middle Georgia) so I listed that as the "City of issuance" on the form. Well my daughter has a new passport, just came today, and hers says United Stated Department of State. When I tried to fill out the online form it ask for country and of course I put United States and then it ask for city. Should I put the city we applied for it in or what?

There shoud be a return address on the envelope your passport came in, of which that would be your city of issue.....At least that's how I understood it.

Ours also says U.S. Department of State so that is what I have been using. The only time I really had to use that was when I filled out our Fun Pass and that is acceptable for that.

We have several different passports in our family. Mine says the city of issuance is Philadelphia and thats what I use. My wifes say National Passport Center and thats what we use for her city of issuance. The DD says US Department of State and thats the city of issuance we use for hers. Never been questioned on any of them....
