EydieG Posted July 28, 2007 #1 Share Posted July 28, 2007 I have a question for those of you who have filled out the online documents and are using a passport. My husband and I have had our passports for a few years and they say New Orleans passport authority (even though we applied and live in middle Georgia) so I listed that as the "City of issuance" on the form. Well my daughter has a new passport, just came today, and hers says United Stated Department of State. When I tried to fill out the online form it ask for country and of course I put United States and then it ask for city. Should I put the city we applied for it in or what? Thanks! My wifes say National Passport Center and thats what we use for her city of issuance. The DD says US Department of State and thats the city of issuance we use for hers.
