Here is my question, I leave this Saturday on the Dream, first cruise for the kids, my step daughter is coming, and her last name is different than mine. Her mother, my wife and I are concerned we will have troubles upon check in. If I were to have a letter of consent written and signed by her dad, would this be ok? also should it be notarized? I checked on Carnival and could not find anything.

Yes you will need a consent to travel from the parent thats not going, and it also needs to be notarized. They might ask for it or not. I always feel it better to be safe than sorry. There is a template on seauonboard.com

Here's what I found on Carnival's website:

 

When traveling with a minor and both parents/legal guardians are not cruising, we strongly recommend bringing an original, signed letter from the absent parent/legal guardian authorizing the minor to travel with you. This will expedite processing by the Department of Homeland Security. Please note that a notarized letter to this effect is required if debarking with children in Mexico.

As long as she is under 16, it is technically required by many nations as part of an anti-parental kidnapping convention. It isn't always enforced, but if you are good terms with her father and access to a notary, it's a good idea. Remember, he must sign it in front of the notary.
