I have looked online and can find the forms for a name change etc. but nothing says change of address. Do I just use Form DS5504? Thanking you in advance for you help. We don't sail until November 20, 2011 so I have plenty of time but want to get this taken care of now and definately not last minute. tnincy Posted May 20, 2011 #3 Share Posted May 20, 2011 i don' think there is a form. i had wondered and searched this myself as i recently bought a home and moved. your passport does not have an address on it. i never worried with it as i figure our government knows how to find me. sorry i know that's not much help.
