Thanks for any input! :) Every major insurer I'm familiar with allows non-related traveling companions to be on the same policy as long as: 1) They reside in the same state 2) Their travel dates are the same However, some will only accept one credit card per policy so if you each want to use your own credit card to pay for your portion of the cost you might have to split up on two policies. If you're on the same cruise booking being on two policies will have no downside. It may even be to your advantage as some insurers have some of their benefits on a "per policy" basis, not "per person". For example, with CSA you might be getting $50K of medical coverage per policy. If there's two of you that has to cover both of you. Or split up on two policies and that becomes $50K per person and the only additional cost is the extra $6 per policy admin fee you're paying.
