Permission To Travel Form

I am asking this for a friend, who is new to cruising and does not come on the boards. She is taken a 2 day cruise to no were in a few weeks. She will have her two grand children with her {6 an10}. They will not stop at any ports, does she need to get a permission to travel form from the parents fill out. Thank you. Teresa

I think common sense would say no but what the cruiseline requires is another matter. I guess it's better to be safe than sorry and take it.

This form is perfect for what you need. Just remember it needs to be notorized before a notary public. And as the form states, "When The Form Is Completed, ONLY SIGN It In The Presence Of A Notary Public!"
