Substituting passengers

I'm booked on a June Summit cruise to Bermuda. My travel companion may not be able to go due to a family illness. I have travel insurance but wondered if it was possible to substitute another person on the reservation?

I was the primary person on a cruise booked in January 2013. I added a second person later. However, due to a medical emergency and hospitalization, I could not go on the cruise. Even though I booked the cruise, the TA had no trouble substituting my travel partner's husband to go on the cruise.
