moki'smommy Posted May 2, 2016 #2 Share Posted May 2, 2016 There is no fee for any name change provided it is done 31 days or more before embarkation. 30 days or less, I don't know since you are not changing people. If this is due to a marriage or divorce, my suggestion would be to bring the appropriate documents to the port. ArthurUSCG Posted May 2, 2016 #3 Share Posted May 2, 2016 Call and talk with DCL. I suspect you will need ORIGINAL NOTARIZED paperwork and not photocopies detailing your name change. Especially if there is miss matches with drivers license, passport and different last names of the children traveling with you. It is a lot easier to collect the paperwork now then to have a family member run around back home to fax copies. Think of the paper work as having an inflated spare tire.
