SNUBA (not Scuba) thru HAL

For those who have done this excursion, do you have to fill out some sort of 'health form'? I'm planning on getting a note from my cardiologist's office (I have atrial fib) that 20' or less is OK for me. Thanks for any help!

When I went SNUBA for the first time it was a breeze. There is a short helath form to fill out. You will only dive about 20 ft as that is the length of the hose that is between the diver and the bottle of air on the raft. Good luck and have a great time. I did. I have since SCUBA dove. and it is about the same only you can't go as deep with SNUBA

On the health form, did they ask questions that I would have to answer 'yes, I have atrial fib'. If I don't have a note saying SNUBA is OK, would I be disqualified from doing SNUBA?
