Greek PLF

I'm trying to figure out how to fill the Greek plf out for cruise stops (we embark in Rome).  Can anyone that sailed already tell me how they did it?  Did you just use your first cruise stop?  I can't see where to note which stops.  Thanks!

They only require your location for the first few days so it depends to some extent on how your sea days fall. For my Greek cruise I just put the cruise ship name and the port address for the location. I figured that was enough to let them track me down if needed - no-one queried it.
