Passenger Locator Form Portugal

We want to go ashore in Lissabon when the MSC Grandiosa is there. Do we need to fill in the Passenger Locator Form? Because if I open it, it only requires information about flights and seats in the airplane etc...it's a little bit confusing.

Not that I'm advocating not filling in the PLF. I can give you my recent experience of it.

Was on the Grandiosa April 5 to April 12, From Italy, to Spain to France and back to Italy. At no point was the PLF checked or asked for, even on my flight to Italy at the beginning, and never when leaving the ship or boarding the ship at all the ports.

Admittedly no Portugal visit, and as I stated I'm not advising not to fill it in, but don't be surprised if it never gets checked.
