2007 Med Cruises -- tipping question

On board tipping isn't a problem because I'm sure Disney will tell us the "recommended" tip level, but what about shore excursions? ...and what if you hire a car/driver on your own? Are tips expected in Europe at the same levels as in the US?

i.e. 15-20% for taxi drive, restaurant server, etc.

Do you tip shore excursion guides? ...and at what level?

In looking at other cruise lines' sites, it looks as if some of the shore excursions might be in the $250 to $400 per person. What's an appropriate tip level for good service?

Since we've never been to Europe before, I'm totally clueless.
