Fun Pass/Passport question

I want to finish filling out my fun pass, but I am stuck on one of the questions. After selecting the type of documentation that I'm bringing (Passport), it ask for Country of issuance - USA, then it ask for city of issuance - this is where I am stumped. I can't find a city, anywhere, in my Passport. I tried leaving this blank and then filling in the date issued and expiration date and then hitting continue, but it says I need to put in a issuance city. Does anyone know where to find the city or is it my address city? Thanks!

[B][FONT=Comic Sans MS][COLOR=darkorchid]When I did my Funpass both last year & this year(for all 9 of of our party), I didn't fill in any of the blanks at all on that page because I wasn't sure what to put down & all was fine.[/COLOR][/FONT][/B]

You can just put whatever it states on the passport. It may say "Department Of State" or "National Passport Center", etc.

I put one on mine and the other on my wife's (whatever was written on the passports, can't remember which one stated which), had no problems.
