Caribbean Excursions with 3/5 Year Old

My two daughters will be joining my wife and I for their first cruise in a month. I have a couple of questions regarding excursions, particularly for St. Thomas and Tortola.

 

1) When Carnival does not allow me to purchase excursions for my 3-year old, is it because she is free, or is she not allowed? Obviously I wouldn't be considering anything beyond island tours/beach time.

 

2) What are recommended excursions for St. Thomas and Tortola. For San Juan, we will probably just tour the old city on our own.
