You do not have to pay more for this service. On some cruise lines it is available for all passengers regardless of stateroom category and on other ships it is available for selected passenger's based on their stateroom category. Though the concierge they can make reservations for you for private tours at ports that you will be visiting or they can make car arrangements for you and give you recommendations just like you would find in a hotel with this service.
