How do upgrades work? Do you just keep checking to see if the price goes down on a cabin that is better than the one you have booked, then call? Or does RCI notify you that they would like to upgrade you? Does it matter what your C&A status is?

Many different ways upgrades come about. Like you say watch for a price drop on a better cabin and call RCCL or your TA if you used one and move cabins and pay the difference. This is likely your best way to get an upgrade...

 

Diamond Plus get upgraded when cabins are available. Well documented formula outlined at length in other posts.

 

Random upgrades just occur. You may not even know until you check in and they tell you no, your not in steerage and hand you a silver or gold key. Just don't expect that kind to happen.
