Parental Consent Form

Several months ago I saw a consent form on one of the boards. It is the form needed for a child traveling with one parent or with someone other than their parents. Does anyone know where I can find a copy?

Our daughter brought a friend along on our cruise in April. On the RCI website and in the travel doc. book there is a page that tells you what needs to be stated in a consent form, we typed the paper ourself and had both her parents to sign it and had it notarized. The airline said since she was 16, nothing was needed on Delta, a 15 year old can fly alone. We had no problem at all.

It can be very simple. Mine is handwritten and notarized. No one has ever asked me for it, but if they do, I'll be prepared. As a matter of fact, we haven't heard from the jerk since I took him to court to make him sign the "form" 4 years ago!
