It is the form needed for a child traveling with one parent or with someone other than their parents. Does anyone know where I can find a copy? On the RCI website and in the travel doc. book there is a page that tells you what needs to be stated in a consent form, we typed the paper ourself and had both her parents to sign it and had it notarized.
