Flight insurance

Do I include this with trip insurance/cancellation (I'm still doing research on all the great links provided here, such as insuremytrip, etc.), or do I use the airline's insurance?

It might depend on the plan you're considering and your needs as far as pre-existing condition coverage or Cancel For Any Reason coverage. For example, this is Travelex's requirement to get coverage for pre-ex conditions:

"The Pre-Existing Conditions exclusion is waived for You if You enroll in the Plan at the time You pay the deposit required for the Covered Trip (or within twenty-one (21) days of the initial deposit) and You purchase the coverage under the Plan for the full cost of the Covered Trip."

And for their Cancel For Any Reason coverage:

" . . . b) You insure all prepaid Covered Trip costs that are subject to cancellation penalties or restrictions; and . . . "

So if you insure your air tickets thru the airline and don't insure them through Travelex you may be forfeiting your eligibility for either of these two benefits. If you don't need either of those benefits then it wouldn't matter.

Choose your best option for covering the cruise and call them to find out what they say. For example, CSA doesn't care how much of your trip you insure for the pre-ex coverage but they do care for the Cancel For Any Reason coverage.

Buying insurance from multiple sources for the same trip usually results in overlap (and/or gaps) and increased premium cost over buying a single comprehensive policy. For instance, both policies would likely include baggage loss/delay protection. You don't need two policies for that, each of which would probably insist that the other one pay first.

Most (all?) 3-rd party travel insurance companies will let you add your flight to an existing policy for nothing more than the cost of the premium bump (which is less than buying a whole separate policy.)
