I've noticed a few hotels startint to charge for a shuttle to the pier. You'll probably find the best and most accruate information in the Florida Departures area of this board. I'll also add that if you don't have too restrictive of room requirements, this is a great area to use Priceline to save quite a bit of money ... part of which could be put towards cab fare.

I got the Hyatt Summerfield off priceline. free breakast, and snacks/drinks during the weeknights. they have a free airport shuttle, but no port shuttle. but I got it for $45 a nite, more than half off the normal price- so I can afford to take a cab!

Cambria suites has a free airport to hotel and hotel to cruise port shuttle. The reviews on the hotel are really good and we have stayed there twice and were happy both times.

The Renaissance is the closest on 17th and Eisenhower. They have a shuttle for $6 pp but you can take a cab and it is less than $10 incl tip.

I wouldn't pick a hotel just for shuttle service. You can have many more choices of hotels and rates, if you leave the shuttle out of the mix. A cab to the port is not much more than $15 from most hotels in the area.

 

Much depends on what you want to be near and how long you will be in the area after arriving.
