Child traveling with one parent

A while back I saw a post giving a link to the form that's needed when a child travels with only one parent. I found one link by using the search feature, but it no longer exists. Does anyone have a copy of the form they could post? We're planning to treat our DD and grandkids to a cruise in celebration of her college graduation and becoming an R.N., but afraid to book anything until the permission from their father is in writing. Thanks for any help you can give me.
