Taking Grandchild

will be taking 5yo grandchild, last names differ. She has her passport. What other documents/letters will we need. TIA

Here you go just print it off take to your local notary and have it filled out. It also includes consent for emergency medical treatment.

We use our bank since they do not charge a fee.

We have travelled on CCL with grandkids often. My daughter's a lawyer and she writes a letter stating permission for out of the US travel on Carnival for her child with dates listing the ports of call and gives medical permission. She and her husband sign it and it have it notarized.We have never had a problem with travel with our grandkids. We do buy the additional travel insurance for the kids too.
