There was a lovely three piece combo in Medusa on the Jade for that, peeking at some older dailies I did not see any such arrangement on the Spirit. Even a duo can work. They leave from the centre of Livorno at 9:30 and you can get a coach transfer from the boat to Livorno centre for $5. They do bus transfer to Florence or even better a mix or Florence and Pisa. We used them on 20 November 2016 and were very impressed with the service. We had 4 hours in Florence and 1 hour in Pisa. I know others had issues with the frequency of the train service but this may be due to the fact it was a Sunday. As for Rome it is a very easy train journey with frequent returns. This will also be a bus transfer from the port but this is a free service in Civitavecchia. From the bus drop off point you can then choose to walk to the train station (15 minutes) or take a 2 euro bus ride (this is well signposted at the drop off point). For Florence we started at Accademia (tickets prebooked through tickitaly.com) we then walked through the city to see Duomo, piazza Della signoria, ponte vecchio. We didn't enter Uffizi but would have had time had we wanted. Instead we spent the rest of our time having a leisurely lunch and generally wandering. It is a small city and everything is close. For Pisa one hour is enough to walk around the field of miracles and take a few pictures of the tower. As for Rome did Colosseum tickets again booked through tickitaly (spent around 2 hours there), roman forum, palatine hill (around 1 hour) and then did a walk to piazza navona, pantheon, Spanish steps and Trevi fountain.
