? on Deluxe Celebration Package

Has anyone ever purchased this Deluxe Celebration Package. We are on Equinox and celebrating out 25th anniversary. My question is, for anyone that has done it, how do you pick breakfast, cake and time of delivery. There is no spot for that on the order forms.
