Vaccine info upload

Received an email requesting vaccine info. I thought I did this previously. However,when I click on the link in the email I get a firm to complete. The problem is it doesn’t allow me to enter my birthday. When I click on the box for my birthdate it brings up today’s calendar. See photos.

What happens when you touch the blue arrows, especially the one to the right of "November 2021"?

I just completed this email. Just click on the the blue arrow next to "November 2021"  and change to your birthday.
