Does Hertz have a shuttle from cruise port to airport to pick up a rental car in Hilo?

No.

No one does post Covid in Hilo as far as I know.

There are a couple of small, local rental places you can walk to or the Hilo Airport is about 3 miles away. All the Majors are at the airport. A taxi may be easier than an Uber.

Budget, Avis and Payless have a shuttle to cruise port from airport rental.

They pick only people with reservations for these companies. and absolutely refuse people from other companies. Actually the security at port exit send you to the taxi stand for other rental companies.

Shuttle runs every 40 minutes or so and last shuttle from airport to cruise port is at 4PM

Also, if you have more than 2 people in your party, they don't allow more than 2 on the shuttle in any direction. You will have to pick-up the other people from cruise port after getting the car or drop them off before returning the rental.

If the shuttle van is not full, you may have a chance to get the other people on.

Confirmed, we were there last week. More people than seats in the shuttle.

Useful to know thanks folks, I might switch my booking to Avis from Hertz. The local companies are difficult because I don't have a US or Canadian insurance policy. Two other questions, is there any parking near the port? Certainly doesn't look that way, was hoping for a 2-day rental but might have to switch to one day. Also, anyone who has booked with Avis what was the wait like to get a car? I have Avis Presidents Club if that helps.

Yes, there is on street parking right outside the port, it doesn't look that way in the day time, but at night its lined with rental cars from the ship. The Avis shuttle driver told us to park on both sides of the road overnight when we drove out of the port. You will see that there is a little park next to the ABC store, you can park on both side of the street from the park up to the port entrance on that road.

As for the wait, it helps to be at the front of the line, we were in the gangplank line about 10 minutes before it opened and were in the port area milling about when the Avis shuttle arrived, even though we were some of the last folks on the bus, were able to grab open seats towards the front, so we were 2nd off the bus and second in line at the counter.

Interesting, just looked on Apple and Google maps and can't see an ABC store. Are you referring to Hilo @DCwom ?

I believe he is referring to Nawiliwili, Kauai. There is overnight parking behind the ABC Store in the Anchor Cove Shopping Center. The parking is next to a public park. It can be hard to get if there is an event going on.
