Based on recs here, we did the Damajagua falls tour out of Puerta Plata when we were on the NCL Breakaway.  IT WAS AWESOME.  Our guide (Jeanette?) was wonderful and the whole trip was great.  We had a group of 7 and we were the only people on our nice a/c bus.  The driver was safe, and the guide was constantly telling us about the local area as we took the 1/2 hour or so trip to the falls. We did add the $10 each for lunch, which was well worth it (quite tasty buffet at the falls site).  Cost was $60/pp which seems reasonable.  She did the falls with us, and took pics for us.  On the way back she offered us the option to stop at a coffee/chocolate demo, which we did (it was cool) but we didn't stay long at our choice, and there was no pressure. The falls was well worth it, and super cool (you can find more / better reviews of the falls, I just wanted to specifically mention our tour group).  They also gave us samples of the local drinks.  Highly recommend this group to see the falls!!  We tipped very well because the service was awesome and the price was reasonable.
