If you have a birth certificate issued by the Jersey City/Hudson County NJ office of Vital Stats, you will need to obtain a certified copy of your birth certificate in order to obtain a passport. Here is the link: http://www.state.nj.us/health/vital/jerseycity.shtml My father works for the post office and when he went for a passport they informed him of this last year (wouldn't accept his gov. id either). It took us a little longer than 3 months to get the certified copies. My dad was funny, he wondered how come the gov would believe who he was when they wanted to draft him during the Vietnam War but not now, lol. ps sorry if the title is misleading, I forgot to put in "Jersey City" and couldn't edit the title.
