Cunard forms

I had read on one of the messages that there is a form which needs to be filled in upon embarktion of the QM2.There was a sight that you could download it from and have it ready before boarding avoiding wasted time.Anyone have the sight?

It is on the Cunard website, under booked guests. Tickets are not issued without it.

Our Travel agent gave us (along with our itinerary)--a CUNARD "Voyage Information" booklet personalized with our names for Independence Day Weekend. Contains tear out sheets to be sent or faxed for insurance, phone, "white star luggage service" etc. if one wants it--BUT, also contained a registration sheet to be sent ASAP.
