sk8teacher Posted April 5, 2007 #2 Share Posted April 5, 2007 I'm not positive, but I don't know of any special "forms" for this. All we did in the past was type out a "To Whom It May Concern" letter stating that XXX has permission to make any medical decisions for XXX during the period of XXX to XXX. Then we signed and dated it, and had it notarized. I would assume that simply adding in something along the lines of "and travel with said person aboard the XXX" would suffice.
