...is it covered in your fare with the cruise line, or do you have to arrange this yourself?

The only people that I have read having to get these cards are those who have a cruise that starts in the DR and fly in. You may want to check with your cruise line; we didn't have to get one when we were there in March.

Not needed for day-cruisers.

It is not needed if you are spending less than 24 hours in the country, you are considered in transit.
