Parental Consent Form

I saw a link a month or so ago for the link to a form that gives a person parental consent to take a minor out of the country. It looks more and more like I will be stuck working the week of our vacation and I will need the form for my DH and my mom to take my son with them. I did a search and never came up with it. Thanks!
