Release form for single parent

OK I know it has been posted here a bunch of times, but I'm using the 'it's Monday' excuse, and I can't find the form used for one parent taking a child on a cruise (where the other parent signs). I looked on the CCL site, and used the search function here, and can't locate it. If someone has the form or the link, can you please post it?

I was under the impression that you just get a signed and notarized letter from the absent parent. That was my plan. If there is a form, though, I'd appreciate the link as well because I didn't see anything on Carnival's website either.
