They did look at it briefly though. You can do one yourself. The one I drafted (I'm an attorney) basically said: AUTHORIZATION TO TRAVEL I hereby authorize (full name/Drivers' License) to take (child's full name) on the (Cruise Line/Ship name) during the period of (dates) and to disembark in all ports including (name of ports). (Consider whether to add sentence about medical treatment which may or may not necessary or really enforceable depending on what Divorce Decree, if any, says-It wouldn't hurt to have the Divorce Decree availiable if you are already authorized to make those decisions ) Full Name Driver's License/Identification No. Address Telephone No/E-mail Affiant/Notary Public Stamp etc Edited July 16, 2017 by Stallion Link to comment Share on other sites More sharing options...
