We are looking recommendations of what to do in these ports and do we need guides? Mytilini - Patmos & Mylos

Don't know about Mytilini, not having been there. BUT, have been to both Patmos and Mylos. Both are interesting, very different and offers some good options to consider. It is not so much about needing guides, but solving certain of the logistical questions. Some pictures below give a little preview. Patmos is all about history, religion, culture, etc. Milos has unique land- and sea-scapes. Milos is an island that is rich in minerals that produces a special white, very fine, sand-like material.

As Figure correctly details, you need to get up that large mountainside to where the various treasures are located. Since it is a fairly small island, your public transportation options are a little limited and can be impacted by how many ships are in port that day. Ship excursion are not always the best, but they do get you covered on these logistics, provide guides who can explain what you are seeing, answer questions, etc.

THANKS! Enjoy! Terry in Ohio

Inside the church part of the Holy Monastery of St. John in the Eastern Orthodox tradition. It is part of a complex with a nice museum and art collection:

Above where you enter to see the famed cave of St. John, this art piece summarizes this history on the island of Patmos. The author of the Book of Revelation is sometimes referred to as John of Patmos.:
