mamakinrocks Posted July 19, 2012 #1 Share Posted July 19, 2012 Hi! We will be traveling with a non-family 17 and 18 year old on the Celebity Summit. Do we need to have forms/documents to allow medical treatment, etc in the case of an emergency? If so, are these available from the cruise lines? Thanks! ...and hope we won't need them : )
Rare 1025cruise Posted July 19, 2012 #2 Share Posted July 19, 2012 Yes, you need them for the 17 year old. You need a notarized form signed by both parents giving permission for you to take them out of the country and to permit medical treatment. As the 18 year old is an adult, nothing required. beachchick Posted July 20, 2012 #5 Share Posted July 20, 2012 Be sure to also take the info on their medical insurance. Just as an FYI (for anyone who doesn't know): Yes, they should, but not for any treatment on the ship or in most foreign ports. The medical center onboard does not take insurance of any kind or bill insurance or anything else. All medical expenses are billed to the onboard account. They would have to get records, etc. for any medical care and then the parents would have to submit it to their insurer for reimbursement. Ditto for most foreign ports. There are exceptions where reciprocal agreements are in place, but they are the exception rather than the rule. But they should have the medical insurance information, preferably the actual insurance card, with them. If medical care is needed traveling to or from the ship, then it's likely they could use their regular insurance's emergency protocol. beachchick
