Traveling with minor cousin...what paperwork?

In addition to the letter I would also have a copy of her medical card and permission to treat. What I did as well, I made a copy of my DS and DIL's passport showing their signature to go with the letter stating I had permission to travel with my grandkids. I'd rather be safe than sorry.

 

Not sure what "en loco parentis" means but most here just type up a letter themselves stating the obvious. Name of child, parents, yourself, where and when you are going to and how you are traveling, then get it notarized.

It is the legal name of the form you mention. There are many templates that can be downloaded from the internet and I find it easier to locate them if you use the legal term. It gives you the same rights as the parent during a specific period of time. Not only is it used for travel but very popular for baording schools and sleep away camps.
