Bibble Posted September 11, 2012 #4 Share Posted September 11, 2012 In Port of Miami last week, there were no forms. When we went to the agent to get our sign & sail cards he said "And I have to ask you this. Is anyone in your party feeling sick?" Now, I realized that this was a terminal that had no kiosks, and it was all full service at the agent's counter, so it "might" be different in terminals where there are kiosks and all you do is "get" your key from the agent. ccruisequeen Posted September 11, 2012 #7 Share Posted September 11, 2012 Special needs uses a form for diabetes (if you use needles for insulin etc) what size wheelchair you need, and if you have allergies or CPAP equipment. Contact the special needs department for details. They gave up the 5-question form at the ports.
