How can we prove he had the shot??? ~Doris~ My certificate is stamped with the name of the place where I received the shot- in my case it was Passport Health. If you haven't looked at your certificate, look and see if the place is on it. (Don't mean to state the obvious, but maybe you have not thought of that).
