Invitations and insert

I'm considering Doing an insert w my formal invitations rather than RSVP card. Anyone done this? I wanted to just include pertinent info as well as where to RSVP (my email or phone ) and w required info : driver license/ ssn whatever it is carnival requires.

 

Has anyone done this ? If so any wording advice ?

We did something like that. We used a pocket invitation to get all the info to our guest like where to park, when to arrive and the customs info. We put lines on the response card for them to put their full name, date of birth, and license/passport number.

I had an insert with our invitations which was the RSVP cards. For the important info needed, I opted to give out a special email address for guests to email their info to us in order to get on the ship.
