At least take out the insurance before you pay the balance as it could be very costly if you are unable to take the cruise. However, be aware that your declaration of pre-existing conditions (if relevant) is based on on the day the insurance starts, not the day you booked the cruise. Also it is in your own best interests to insure early as payments made for your holiday before the date of your insurance are not usually covered.
