I had printed the form with my boarding pass - it was the old form. We forgot to fill them out before arrival and ended up having to fill it out there - we were given the new form (with the entire right side about pregnancy). When we gave it to the agent at the check-in desk, she gave it a fairly cursory once-over and initialed the bottom of the pages. While waiting for our S&S cards, I noticed a lady sitting behind all of the agents, at a table, unfolding and looking at every health form that had been turned in.
