Health questionnaire available at New Orleans port?

My husband and I are new to Carnival and just realized we only have one health questionnaire in our docs. We sail tomorrow on the Dream out of New Orleans and are not at home.

 

I assume there will be forms available at the port when we check in. Is that correct? I know it will slow us down a bit.

I'm not sure about New Orleans, but have to believe it is the same as Port Canaveral​ and here they have the forms available if you are missing one. They have a little table just before the person who takes them from you.

 

So, don't worry and do have a great time.

if you need copies of the questionnaire, just print out more. all of the forms is the same
