Private Car and Guide in Tokyo

We are looking for a private car and English speaking guide for one day in Tokyo with pickup and drop off at our hotel.  Has anyone had experience with this?  We do not wish to use a guide that utilizes public transportation, as many of them seem to do.  I've seen listings on the internet, but am looking for recommendations.

One warning:  If you use an automobile, be prepared to spend a lot of your time in traffic.  That's one of the BIG reasons that the guides (as well as private citizens) use the subway.
