Luggage tag / itinerary template

Hi sometime ago someone posted a link to a template for luggage tag with itinerary inside. I thought I saved it,  but alas unable to find it now.  Can anyone assist?

Here’s a JPG file that the company who sold me the reusable Princess luggage tag holders gave to customers.  

 

It’s the same size as the Princess tags & tells luggage handlers to open it for information to redirect misplaced bags.  I use Word to put our itinerary on the inside in hopes of it reconnecting with us when away from home.

 

Thankfully it hasn’t been necessary but it’s an effort to reconnect with our bags if they are ever mishandled.

You’re welcome....I’ve seen misplaced luggage tags for purchase on travel websites but that form adds another function to the reusable Princess luggage tag holders.
